New Patient Information
Your First Visit
At Second Line Family Dentistry, we’ll treat you like an old friend even at your very first visit. You’ll receive a courtesy phone call from our office and we will offer to let you fill out your patient forms online. This will help you save time during your appointment! When you come to see us, Melissa, our front desk coordinator will greet you at the door. Then, Tori, our dental assistant, will walk you through the office so you can get to know us and we can learn a little more about you as well. You’ll then meet with Dr. Mayda Ferguson, and she’ll review your medical history and let you ask any questions you might have. From there, she will take a variety of images and x-rays so she can come up with your own customized treatment plan. She’ll take as much time as you need to make sure you understand every aspect of it. From there, we’ll introduce you to one of our hygienists, and they’ll perform a dental cleaning.
For your convenience, we have made it so you can fill out all of our patient forms online. Simply follow the prompts and all of the appropriate information will be instantly send to our office—no paper need! This will help both you and us save time so your next appointment will fly by even faster. Be sure to let us know if you have any questions!
